The United States, Israel, and Iran have never engaged in a prolonged traditional war, yet direct confrontations have happened several times. These conflicts are typically brief, consisting primarily of missile and drone strikes, as well as air assaults. Up until now, five to six such direct encounters have taken place. The most significant of these was the 12-day battle in June 2025.
On each occasion, the US and Israel have inflicted more damage on Iran than they've suffered. While Iran has attempted retaliation, it has consistently faced greater losses. Though these confrontations are brief, they cause significant anxiety across the Middle East, notably driving up oil prices.

The 1988 Skirmish
In 1988, a rapid yet intense conflict transpired between the US and Iran, known as Operation Praying Mantis. It lasted just one day, with the US achieving a decisive victory. Only two American soldiers lost their lives, whereas Iran suffered heavy losses, with 56 soldiers killed, and half of its naval force devastated.
Major vessels sank, smaller ships were destroyed, and oil platforms were incinerated. An American helicopter also crashed. The battle witnessed the US utterly overpowering Iran's navy.
January 2020 Clashes
In January 2020, the US eliminated a senior Iranian commander, Qasem Soleimani. Iran retaliated by launching 11 missiles at an American base in Iraq. This skirmish lasted just a few hours and days, resulting in no fatalities but causing traumatic brain injuries to over 110 American soldiers. Though damage was limited, tensions escalated significantly between the two nations.

The Twin Skirmishes of 2024
In April 2024, Iran barraged Israel with over 300 drones and missiles, necessitating a swift Israeli response. The conflict endured for two days. Israel thwarted 99% of the attempts, suffering minimal damage. Come October 2024, Iran launched 180 missiles, to which Israel retaliated. Again, this resulted in a brief conflict lasting just a couple of days with Israel claiming victory both times.
The 2025 Large-Scale Conflict
In June 2025, what became known as the 12-day war saw one of the most severe confrontations between Israel, the US, and Iran from June 13 to June 24. The collective US-Israel military forces severely undermined Iran’s nuclear capabilities. The conflict resulted in 1060 to 1190 Iranian casualties, inclusive of over 400 civilians.
Israel suffered 28 casualties, while multiple Iranian nuclear plants, missile factories, and air defenses were destroyed. On June 22, American forces struck three Iranian nuclear facilities. Ultimately, Israel and the US emerged victorious from this confrontation.

The Ongoing 2026 Conflict
As of February 28, 2026, Israel and the US have commenced a renewed offensive against Iran. The conflict persists, with its duration and potential damages yet uncertain, situating the situation as extremely precarious. The world watches intently to see what unfolds next.
Throughout history, there have been five to six direct engagements between the US, Israel, and Iran. Each time, Iran has endured greater damages. Though Iran has retaliated, the combined strength of Israel and the US has proven dominant. Despite their brevity, these confrontations instill global tension.
